Good luck. It is really hard. One thing that has worked for me in the past, is frame it in terms of me. "I'm really worried and when you do X it stresses me out A LOT." No ultimatums, but just state that X behavior results in Y reaction by someone who loves him, and cares about his well being.
It may not work, but it's honest and sets up an expectation of your own reaction to his actions, which DO affect you.
Yeah, his response is "I'm not worried, damage was to bone, not ligaments." So... s'alright.
