Zenkitty's right. Early on, it was just his mind. He gave birth in one ep, for example. Later, they changed the "mechanics", and you get things like the double-amputee and the fathering-a-child-who-is-clearly-his. This is also reflected in a change in what Al saw when he looked at Sam.

Did they retcon, or did they change how it worked within the show? Everything I can find indicates it's a retcon, which means that's the way it "always" happened.

My recollection is that it's generally treated (and I don't know if this is in fandom or officially) as "Well, it's a time travel show, so something that he did in the past changed the timeline in such a way that the mechanics are this", which, I suppose, makes it more retconny than not.

What was the turning point? And how in hell is it invisible to so many people? Including me? (IO9 doesn't seem to care that the linchpin of their article is invalid--I'm oddly disappointed)

Cannot remember. There must have been an episode where they first did the he's-actually-physically-there thing, but I don't recall which episode it was, or any exposition or explanation happening.

I didn't watch QL. Reading this discussion, I'm not the least bit sorry. But it has me pondering the difference between a quality retcon and an ass-pull. I know what it means to me, but I'm having a hard time remembering solid examples of ass-pulls that everyone would agree on. I, for one, would consider Wonder Woman's change from using the Invisible Airplane to being able to move about on air currents on her own as a bit of an ass-pull, but that may be because I really, really liked the Invisible Airplane.
