This is my first ballot, so I'm not sure of the procedure. Do I post the final form of the ballot here and say "Let loose the dogs of voting"?

edit: also, I presume the No Preference rule is still in effect.

PUt in a line for No Preference to count towards the quorum. I remember some talk about taking that out but nothing settled.

the phrasing of the ballot is up to the framer (you). you can have it on or not - there is not rule requiring it and no rule disallowing it.
