Well, I'm a psychologist and I think it means what you said. It's the most common form of self-harm in adolescents so people sometimes use "cutter" to refer to all kids that engage in self-harm, even if they don't actually cut themselves. Perhaps that was what she had in mind.

Also, I didn't mean to imply that because she's psychologist that she is all knowing of all things goth and stuff. I just meant that cause she's one, she would know what a cutter is in terms of self-injury and would have been all over that shit if her son said that he was one or a friend of his was.
