It looks like this guy has had some serious complications from the accident, but having some buckshot rain down on you, especially while quail hunting, is pretty common.

He didn't "have buckshot rain down on him"; he got caught with what looks like most of a load of birdshot. PDF of the hunting report here: [link]

That's not "peppered". That's "shot".

Apparently the problem with having birdshot in your throat and chest is that over time it can migrate into the circulatory system and thence to the heart. It isn't that he was shot in the heart, it's that the birdshot is on the move.
