We don't see Harmony die, right? So I don't think she counts.

I was surprised that she was turned. When I saw the bite, I assumed she was just vamp food and would end up dead. Don't know if the Scooby gang saw it. I would doubt it since she got bit in the middle of the big fight.

Personally, I don't think the writers thought about it until they decided they wanted someone for Spike and they wanted Mercedes back on the show.
