I do unlimited for my company. Part of making it work is that it really needs to be modeled and pushed from the top all the way down. Every time we come off a busy period I encourage all of my staff to book the time off. Then we all make sure that when someone is out, we guard that time for them, so they can really relax, and so they aren't coming back to a massive pile of stuff.
I used to get four weeks (and then, briefly, unlimited), but with the current job, they are hard set about 2 weeks. I tried to negotiate based on my seniority, and nothing.
I just checked, and I'm sitting on more than 30 days of vacation and almost 67 days sick. Yikes.
What I said before was wrong, and I can now roll over 8 weeks, not 6.

I cashed out 2 weeks of vacation time this year and last year, which was nice to have the extra money but I think in a general way I would be better served by taking the time off.
