I got nothing for you except to use the pomodoro technique. That always helps me out when I my concentration is fucked.
25 minutes on task, 5 min break. If that is too much, try 10 minutes as a warm up.

Zen, would podcasts help at all? Sounds weird, but when I HAVE TO focus on something, but JUST CAN'T FOCUS, sometimes putting on a podcast helps distract my brain enough that the part that's BORED with the doc I'm proofing (or whatever) is engaged, and the leftover part can keep reading. (Recommended by an also-ADD-friend, though she uses audiobooks. I'm cheaper than her and podcasts are free).
