Generally, it is due to one or more of the following:
1. Thought she was infertile
2. Was using birth control
3. Spotting
4. Erratic or mainly absent period most of the time
5. Large woman
6. Small baby
7. Fetus/placenta positioned in such a way you're not going to feel much movement
8. Some other medical condition to attribute the symptoms to.
That last one is a biggie.
So, like, this one woman was 41, had been trying for years to have a baby, recently had a gastric bypass, had always had irregular periods, and HOLY SHIT she just had a baby.

I was wearing a purple sweater today, but I didn't remember that I had a reason for wearing purple until after I'd already gotten dressed. A pretty significant portion of my clothes are purple already.
The purple I own is linen and today I'm doing things that would do bad things to linen.
My dad is wearing a purple long-sleeved t-shirt today. Just by accident. While I kinda chuckled, I didn't tell him about the purple shirts today. I think most of the women on "I Didn't Know I Was Pregnant" are PCOS -- strange cycles, weight changes, wonky hormones are par for the course AND they're likely infertile.
By month eight or nine, though, when that baby is kicking and rolling and sometimes hiccuping, it's got to be a *really* small baby not to notice it. At least in my experience.
Most of them, iirc, don't go all the way to term.
Of course, it can be tricky to tell since they don't have any idea when the got preggers -- but the babies are on the small side.
