Short version: wincon 2008, everyone concerned was drunk. Thenyxie's husband got confused between online fantasy and reality, and forced sexual attentions on someone(s) who was not okay with it. The someone(s) neglected to say anything until planning for wincon 2010--or 2011, I forget--when extra security was requested, and events of 2008 were revealed. Thenyxie got aggressively defensive, demanded to know the identity of the accusers. This was granted (mistake, in my opinion) and the excreta impacted the ventilating device. Cue escalation, and everybody in the entire world weighing in on one side or the other.
Fandom still in uproar.

Nothing. I didn't read any posts other than the ones specific to SWSNBN. That's something you'll have to do your own research on, I'm afraid. Although, I did see one brief comment about Jared saying he threw a bit of a hissy-fit. What over, I'm not sure, but it sounds like a tempest in a teapot.

Ew. Having experienced more than enough toxicity in another fandom, I think I'll stay way far away from this one.
IIRC, the events were brought to concom's attention at the time, and things were discussed that left concom thinking they'd been clear about her not being allowed back in the future.
That may be in the unfunnybusiness round up. I do dimly recall there had been some murmurs back then, but no names named.
