So Castiel conceded that Sam actually could stop Lilith... at the cost of making him gulp down the demon blood. So theoretically at this point Sam could still serve on the side of good, although after the battle it sounds as though he'd be in a bad way. (I know by the end of the episode villagers will probably be hunting him with pitchforks and burning torches, so please allow me my comforting illusions while I can still indulge in them.)
