I thought he said planet, but then he didn't take the bottle of relief, so I figure not that bad after all?

Maybe they didn't have access to pain meds as hunters and were used to just bearing whatever they had to bear, but Dean had occasion to get a legal prescription some time during his year with Ben and Lisa and that experience convinced him to keep some handy for emergencies. Sam was surprised because that hadn't been SOP before.

Well not canonically, no, but this is transformative fiction.

I said piano? Uh, oops.

I'm sure that if they had access to down-low medical procedures (canon as of Appointment In Samarra there's at least one hunter doctor), painkillers were theoretically available.

Obviously, I'm biased. My world revolves around pain relief.

We know canonically Sam broke his wrist at least once. No ER doctor would have sent him away without pain medication.

I can see Winchesters deciding that painkillers would dull hunting skills too much but they can function just fine on whisky.
