I've been to very few weddings where the only person I know is the bride or groom. Those that I have attended, I did not go to the reception adn my RSVP reflected that.

They did make one inspired by the same source but wearing a paramedic uniform on a recent ep! It was adorable!
I think if I know someone well enough to invite them to my wedding, I'll know their SO's name and put it on the invitation so no "and guest" will be necessary. (Or they're an obligatory family invite, I'll be able to find that info out.)
OTOH, if you don't include "and guest" as an option for your single invitees, they're likely to RSVP for 2 anyway, so you may as well account for it when you're sending the cards out.
