I think that Mom let him out OR Tony left the cage open when he saved Sam but there was all that earth already filling in the hole.
They hadn't totally filled it in at the time. But then, maybe the demons decided to fill it in to cover up the dead human body.

And then I have a wonder: (possible spoiler for one of the most recent eps, but not plot related-more about the Time Lords' capabilities) the Dr can see what can be changed and what can't. He picks and chooses his butterflies to smoosh with great care. And then you have Destiny. What does he do? What happens if he puts the book down so he can go take a whiz?
