Do I tip the manicurist, and if so, how much?

Yes! I always tip %20 for pretty much anything. The math is relatively easy & I don't have to keep different percentages in my head. Of course that doesn't work for tipping baggage handlers ($2 a bag, right?) or housekeeping at a hotel...shit, i'm realizing that I bet we didn't leave a housekeeping tip when I was in NC with my brother & nieces. I was too crazed getting the nieces together.

Yeah, it's the one thing that's cheap about living here.

Also, I always forget to tip at hotels, and only feel bad about it when it's way too late! That's a weird custom, I think -- most other people I'm tipping, I'm looking at them when they're providing the service.
