In the second episode, when all the zombie ghosts showed up because the seal of the witnesses had been broken, who's to say Dean isn't one of them? The greek word for witness is martyr and that is very much what Dean did for Sam, gave his life to keep Sam free. Even the iconography of last season's finale is of Dean apparently crucified on that web. I can very much see Dean being one of the witnesses that got released and Hell is using him to whatever extent it can to manipulate Sam.
Third parties' reactions to Castiel would seem to contradict that, though. While we only have his word that he's acting on orders from God, all the demons in the area went to Defcon One when his presence became known, and he's demonstrated complete immunity to the best warding sigils Bobby could manage and to Ruby's demon-killing knife.
That means he's powerful, it doesn't necessarily mean he's 100 per cent purely Good.
I think he's definitely powerful in a way that is completely unheard of by all these formerly-human demons that have been running the co-op in Hell. Of course, it occurs to me that Lucifer is also a nigh-invincible mythical figure that hasn't been involved in the running of Hell or directly acquainted with aforementioned demons for thousands of years or more...

This would too. When Dean was first out of the grave and in the filling station, I thought he was still in Hell. I've changed my mind as we work through a few episodes, but maybe that's just being lulled into a comfort zone. I need to make a list of things I believe: 1)Dean is out of Hell, 2) Dean is not a zombie, 3) Castiel is a fierce fighting Angel, and should never be confused with the word cuddly, 4) Castiel is a bad ass, and may do questionable things, but for the side of good (id est he's not Lucifer), 5) The reason John was smiling so much when he got out of Hell was because he was John the civilian again, believing in happy endings.
