Is there a general feeling that the Doctor/the showrunners ought to be more careful of Martha's sensibilities/their portrayal of her empowerment because of her skintone? Or would you say that she is generally portrayed, as a character, in a demeaning or racially stereotypical fashion?

I wouldnt' say that. For me, it seems that Martha's race never seems to be an issue or noticed, even in times when it should be. OTOH, her clothing never seems to be an issue either. They vaguely acknowledged her race in The Shakespeare Code, but no one commented on the fact that she was wearing pants, which I found odd.

That is a pity. I was glad that Rose's desperately inappropriate garb was commented upon in the Werewolf ep - that whole "Wee Nekkid Child" thing. (Although it bothered me that there wasn't MORE made of it, rather.)

I'm pretty sure that race relations were different in the two periods. Not that I don't think the writers were anything but lazy, but the idea that she'd generate two very separate reactions is quite plausible.
