If I'm at home I've a secure location (read: buried in piles of unrelated crap) where I write most everything down.
Heaven help me if I change a password at work. And the work passwords? Let's just say my life got a little easier when the sysadmin moved into my office. (I don't actually forget work passwords often. I do lock myself out with typoes, though.)
I'm awful. Golf cart going over cliff made me laugh. It's the Jay Landsman in me. "Hell of a handicap, bunk." "That's it...arrest Arnold Fuckin' Palmer." Etc. Etc.
I have Keyring for the "important" passwords--FTP, stuff like that.
Site registrations I have a system for. However, when I merge two accounts, I may be merging two unmatchy instances of the system, and that's where I get confused.
Plus I always think I'll remember.
