The first aid class that I took was pretty clear that you should never use a tourniquet unless absolutely nothing else worked, it had been bleeding even through the pressure and elevation for a good while, and there was absolutely no way to get to a hospital. (Pretty much all first aid courses now are focused on learning how to figure out whether an injury requires medical attention, and how to stop the injury from getting worse until the medical attention gets there. You've got to take special "Wilderness First Aid" courses to learn stuff like how to wrap bandages and stuff like that.)
