All we're really looking for is a simple way to let the person getting the warning know that it's official, and the person issuing it has the authority to do it.

I like the idea of 6, but I'm not sure about the "this is a final warning" text. I'd think that that would be in the text of the post. Maybe something like that, but without that text, for a warning, and then similar things that said "Suspension" and "Banning" in the Buffy font for those. (Are suspensions and bannings reported in-thread like that? I can't remember what was decided there.)
