My first real world meeting of people I knew online would have been people from rec.music.gdead providing crash space during a run at Cal Expo. Looking back, that was fairly insane, but I did go in the company of a couple of guys I knew from college. It was more insane for them - opening up their homes to people they'd never met. It was a great time, though, and I ended up seeing a lot of shows with them. I'm not really in touch with that crowd anymore, alas. Before that the people I talked to on BBSes were people I knew in meatspace first.
