You should. Write up at least the bullet points of what you want to say, and mention the illegal aspects of how you were treated.

Also, maybe write a formal letter of complainted, with a copy sent to the ADA. (And make sure they know there's a copy sent to the ADA. Sure, you'll never be going back there, but that's all the more reason to make them squirm.)

I wouldn't. Almost nobody gives bad ones anymore because of liability risks. THey pretty confirm dates.
