I'd never read Mark WAtches/Read before this. I'm not sure if he's ever dealt with characters that were not sane. I get that he doesn't like the word crazy nor does he want to use it. From what people have said here, saying someone is insane is okay, or at least acknowledging they are mentally ill is okay.
However, there's a big difference between not using a word you think is a slur and ignoring a huge aspect of a character.
It's going to feel really insulting if he's flat out refused to acknowledge her mental state.
But I'll give him the benefit of the doubt and say that maybe he's not sure exactly how much of a role Dru is going to play. Maybe he thinks she's just going to be Spike's "strange and awkward" girlfriend who is also weak and "helpless". She did turn whatshername in School Hard and tried to eat a kid in Lie To Me, but she hasn't been really threatening other than that and has just been in the "girlfriend" role.

Willow’s beautiful freak out on Cordelia and Xander and her demanding they leave HER library is everything that is right with the world.
This really is a wonderful Willow moment.
