They've brought on one-off actors back to play larger parts later (Gwen, Martha), so Billy wouldn't be out. Billie, however...although, Romana consciously regenerated to look like whatever-her-name-was, so it would be possible, I guess. I can't really imagine the Doctor choosing to be female, though.
