Oooh! Hill Street Blues had a great "death." In its pilot one of two imperiled officers had died, but when the show was picked up the character had been so well received he pulled through. [link]
Then there's Roseanne I & II where they killed Dan Connor because John Goodman wanted to get going on his film career, resurrected him for the sequel, then killed Roseanne when she shot her mouth off and made the show The Connors... which harkens back to Valerie/Valerie's Family/The Hogan Family where Valerie Harper annoyed someone and was killed off. And, of course, Eight is Enough which was supposed to be about a big happy stable family and then Diana Hyland died, recasting her was deemed evil, and they made him a single dad with a girlfriend.
