I think only one per show, so can't be Tara. Quentin is a good choice, as it kind of killed off the show as well.

Eva Green's Vanessa Ives on Penny Dreadful should count, right? A ruinous ending to a great show.

Sorry, but it was, "Let's be careful out there". We watched this religiously at the time. Esterhaus's death was a real, huge shock, second only to Henry Blake's in M*A*S*H. The difference here being that it was actually the actor who died.
