If I don't 100% know, I err on the side of tipping. (To avoid the aforementioned jackassery. As well as nincompoopery.)

as far as I know medical massage means no tip and the hospital around here that dies them is supposed to be really good
Agreed on the sole proprietor/owner thing, in theory (but agree that it feels weird sometimes).
Really? I've never heard of that outside of servers. (Farm workers are another category with reduced minimum wage reqs, but not really relevant here.)
Farm workers?!? Really, what's the justification there? They're not getting tipped! And they're working really hard!
That said, in some places the waiters get minimum wage (here in Washington) which means with tips, those are pretty good paying jobs (relatively). Thus all the people who move here, become waiters and bartenders and baristas while pursuing some other interest.
I suspect it's good that I've never had to rely on a wait staff job, as no matter how good the tips are they wouldn't cover bail after my reaction to some of the customer behavior I've seen from fellow diners.

Not the minimum wage thing, but for most baristas, hair dressers, manicurists, hotel staff, and casino dealers, 40-50% of their income comes from tips. For food delivery, I believe it was far higher.
Personally I find the whole tipping system ridiculous, but now that I know that reality, I work with that frame of reference.
Jesse! Paging Jesse!
Speaking of doctors, I'm going to the eye doctor today, and am seriously considering Zenni for new glasses. Has anyone actually used them? Anything I need to know?
I've used Zenni and I love them. I have like 7 pairs of glasses that I have gotten there. But my PD measurement is perfect and my astygmatism mild. LOVE.
So, this is why I get mad at CNN and think it is worthless at times:
How did Wolf Blitzer keep a straight face?
I order from Zenni, as do several of my coworkers. The trick is making sure you get your pupillary distance along with the prescription - many optometrists won't include that because they assume you'll be buying your glasses from their affiliated brick and mortar shop who will take that measurement when you order the glasses. I didn't have that on any of my prescriptions, so the first time i ordered from Zenni i had go to a glasses shop when it was empty and ask one of the employees really nice to tell me my pupillary distance.
Just got my most recent order (2 pair, frames, prescription lenses, anti-reflective coating, cases, shipping...all for less than $35 and arrived about a week after i ordered them) and i LOVE one pair. The other was a whim - i wanted to try out some chunky, rectangular frames and they turn out to be not especially flattering. BUT now i have them for Halloween or any event where i want to look like a hipster. And for the rest of the time, i have my old reliable half-wire rims.
They don't really have a return policy, so you get what you order and that's that. But for the price? I feel like i can afford to take that risk. Works best if you already know what kind/shape/size of frames you want - then find them on the site. They do have a new option of "modelling" the frames on one of a small selection of faces which helps me better understand the sizing aspects (the pair i ordered a couple years ago ended up being way too wide for my face.)
Zenni won't fill my prescriptions. Beyond what they handle.
