Bottom line, as long as the work gets done on time, no one really cares.
Same here. We try to keep regular hours so we know when people are available, but no one's punching in and out.

In a related note, we interviewed a candidate today, and I noticed from her CV that she's at the same company as a former co-worker of mine, so I messaged her and asked if she knew said candidate since we were interviewing her.
Oh, she said, that's why she's "working from home" today.

I got asked that once, very casually, and I said "He doesn't know, I took the day off" When I talked the the hiring manager later, she told me that the guy asked everyone that and if they said that they had lied at all, they were out of the running.

For me, it was very nerve wracking and stressful. I guess it depends if you like your current job or not. If you are looking to escape, I imagine it's a lot easier.

Huh, interesting. I mean, I'd feel really uncomfortable about it, even if it was necessary, you know? And yet it would be

uncomfortable to straight up tell your boss you're looking for another job, right?
In this case, I just go. In former cases, I would take a half day or a long lunch if possible. And sometimes they know that you're interviewing.
Since ours is called "paid time off" so being sick is the same as vacation, I tell my reports that I don't need to know which they're taking. It's their business. I tell R that if he calls in with a migraine and it goes away in the afternoon and he can get a golf game in, then he should!
I wouldn't necessarily refuse to hire someone because they had to tell their current employer a lie to get off of work. Some places are really crap about giving necessary time off - it might be one of the reasons the employee is looking elsewhere!
