So it's looking like Fitz realizes, on some level, the Simmons isn't actually there (though I was amused that his hallucination felt the need to argue for her existence to people who couldn't hear her.)
I think that the two together make Andi's case for his brain doing it as a coping mechanism while he heals.

I like that Fitz has someone who, if not his other half, can work with him to get things done.
And Phil has May to help him with his "episodes." I loved the ritualistic aspects.
Why have him use a knife, when chalk or even permanent marker would be easier on the hands?

I thought the fact that Skye mentioned that when the symbols showed up on the obelisk they were also tactile - she said they were "like braille" - that it's intentional that the guys keep carving, rather than writing, them.
