Robert Culp was on Columbo 3 times, as 3 different characters. Soon-Tek Oh played 5 different characters on M*A*S*H. I'm sure there are others.

There's always Jeffrey Coombs and his Star Trek run--he played two different characters in one episode of DS9, plus appearing on Voyager and Enterprise. Nine roles total.

And there's a different sort of record for Richard Belzer, who's played the same character on nine different series.
