Bev, my knee-jerk assumption was that
wherever they are, Dean is the lead player. Probably because Zachariah is trying to manipulate him into becoming the vessel. Sam maybe even isn't supposed to be in the fantasy, or they're not showing his portion of "heaven" in that clip.
But, it's a trailer, so it's hard to do more than guess.
For me, the clip Bev is talking about was so fast, I never even noticed any of the things she mentioned way back, like
Dean's shirt
or any of that.

Okay, there's a little bit of Mom in the first one. I've watched them all out of order on youtube, so I'm totally confused now. Since it's based on stuff conflated from all of them by now:
if Sam dies first, it should be his heaven they're both in. Driver picks the music, shotgun shuts his cakehole. But heaven is Dean's deal, after all, so if it's a Zachariah trick (which I don't want it to be--I want it to be real), I guess it's more about where he would go. But where did Sam go before Dean died? Why did Sam join Dean after Dean died? Are they really that codependent? Okay, last question mostly rhetorical. I'm sure they hunted each other down at the very least.
