Yeah, I'd imagine Rupert Giles would have been in a managed care facility asking a nurse to remind him he likes jello by Season 3 if head injuries were treated more realistically on TV.

He magically healed up long before that. The scars on his face from the car crash and the shadow attack come to mind. Vanished without a trace.

It would have been cute if End!Dean had had a dramatic facial scar.
