I think he was dead 2 days our time. That is, earth time. I don't know what that would translate into in hell-dimension time or heaven or limbo timelines.
Dean was gone for three months earth time which was 40 years in Hell. I am sure that someone somewhere has done the math for Sam's time in Hell. Surely? Huh. I assumed that was where he was but just didn't remember or was able to push it back by the same method that let Dean deal.
Dang... Next week is going to be deliciously painful, isn't it?
I was thinking the same thing and then I was thinking that I am so very worried for us and what this show has done to us.
I can only remember Sam's reaction to Dean's post mortem tales, but it seemed surprised to me.
I'm very invested in Winchester pain. I don't want them happy until after season 6.
I really don't believe Sam was in hell. He was murdered, and I think given the good he'd done, I'd go with either heaven or purgatory since Kripke is using the Christian construct.
I think it's more important that it's never come up, and if he had been in hell, I think we'd know it by now.
Assuming the taint of demon blood wouldn't keep him out, but I don't think even Kripke could be that unfair. I think we're cheated by not knowing by what happened to him, but I don't think he was punished.
Dean paid with hell for his deal, not his life.
I just have a mental image of Sam sitting on the too-low couch in the waiting room from Beetlejuice, tapping his fingers on his knees and listening to awful Muzak.

JDM was a series regular on some syndicated show called "The Burning Zone". He played a doctor who, along with his CDC-like colleagues, investigated viruses and other biological and medical threats/weapons.
Given how many ghosts and whatnot we've seen, and the episodes featuring Reapers, it seems that lots of people's afterlife options take a while to go into effect, and the process isn't without glitches. In fact, I don't recall seeing evidence that anyone who hasn't sold their soul to a demon ever goes to Hell.
