Also, it's unlikely your head is going to stay perfectly straight unless you make an effort. Which you can't, if you're dead or comatose.

I don't think your head will tilt if you're dead, and when I pass out from drugs, I'm straight up.

But I like the idea that Dean tilts Sam's face so he can look at him from the other room, and that he has a specific way of arranging his brother while he goes to do something stupid. Or waits to see the results of the last stupid thing he did. Because it's a habit by now.
