Possibly Anna was the first to recreate the earthly vessel which had been destroyed by heaven's hierarchy. Finding herself outside the customs and protections of angel-hood, she may have defied some written or unwritten law in reforming a vessel with which she was familiar and of which she was fond. Or she may have done it out of rebellion, and not wanting to likely ruin another human by wearing her (or him).
I can't think of a good reason to reconstitute Jimmy's body for Castiel, other than actor continuity. And I am still surprisingly resentful that Jimmy's body has been recreated for Castiel's use, and Jimmy and his family were collateral damage. It's like the writers made him just for Dean, and it makes my skin crawl.
But I do believe the whole "angel condom" aspect of angels in the SPN verse is the main reason angel/human fic gives me hives. I could see it between Anna and Dean that one time, possibly because she was new at angelhood and still very human in her interactions. A return engagement post-reconstitution, with her fully aware of her angelhood would be as creepy for me.
I emphasize the "for me" in both cases.

I've also never had a problem with the idea of Castiel integrated into a Jimmy-free body. It's just that with the two of them walking around like that, what's Michael and Luci's problem?

The inconsistency of some of the rules drives me nuts. It's like Castiel saying early on that several of his garrison lost their lives in battle. Wait, what? I thought only angels could kill angels, so how did they die?
Vesselhood has problems, but when it comes to Jimmy I like to assume that post-explosion in 4.22, he was sent to heaven. After all, Cas would have gone back until God decided to send him back to earth. That he sent him in Jimmy's body I just handwave as a favor of familiarity for Sam and Dean and even Cas.
Anna calling in favors to get her preferred body back is more problematic for me, because who exactly granted them? God? And it does make you question why Michael and Lucifer can't magic up a body somehow.

Honestly, Castiel is Misha for us. It's that simple. But they needed to talk around it more.
Well, that too. It doesn't bother me to understand the meta reasons behind decisions, although once in a while it bugs. Like in Skin, how does the shapeshifter also replicate the victim's clothes? S/he should be showing up naked and rooting through drawers, but I get why they can't do that on TV so I just ignore it.
And I for one am happy as hell to have Misha. I think he and his character have been awesome.

A large part of Castiel fans accept him as slash fodder with Dean. While I'd have no problem at all with Dean/Jimmy, other than the human moral issues of unfaithfulness, the otherness of angels is proving a squick for me.

As I said, it's a personal thing, I don't expect anybody else to share it. I've attempted to get past it, but it's stubbornly right there, making all that yummy TFW and spn_foxhole fic unpalatable to me.
