My bet is Sam is coming back evil, and Dean has to fix it

Except, as much as I enjoy watching JP acting all evol and stuff, haven't we just finished a long, long arc of Sammy being darkside and Dean worrying over him and trying to push at him and fix him? Wouldn't it just be more of the same?

But we went through that with Spike on AtS. Do you think they will try that because it got old very quickly and eventually demanded corporealizing. It just seems so gimmicky and trite that I hope they avoid Ghost!Sam.
