I'm 100% sure that we're getting Sam back, not something Sam-shaped. It might take a bit to get back into the groove, but the show is about brothers, and if that's not really Sam, this isn't really Show. So Sam being dead and this being something riding his facsimile? Really doubtful.

I'm mildly guessing ghost!Sam. Body went to hell. soul didn't. Soul without body equals ghost. But no strong attachments to theory. As I said when I originally proposed it - just a guess, lots of other explanations.
