I know I'm always wrong about these, but has Apollo ever been human besides the time he was unceremoniously killed in the first round (and replaced by Starbot)?

He has! In games 3, 9 and 13. Interesting trivia: the humans won each of those three games. The Cylons have won every game in which he was a Cylon (excepting the current game, of course, on which the jury's still out). I would need to check further, but I think that makes him easily the most successful character on the Phoenix board. (On boardgamegeek, his performance is much closer to average.)
