Weird! I just saw a mic on Book!
When he looks at Simon as he comes into sickbay, there it is, big as life, on the right side of his jaw.
Can't rewind to see it again, but I never noticed that before. Why would they have a body mic on him, especially in a scene where he has no dialog?
Nathan's eyes were so pretty when he said he's a mean old man.

Bit of a test, I'd guess, see if Simon actually gave a rat's ass about his patient, but that doesn't completely account for it.

That does make it a little better for me.
I'm fine with the moral ambiguity thing, but Mal being mean just doesn't sit well.
