JA's commented on how difficult AHBL2 was for him, how draining. Neither of them have said anything about filming this episode. Or that scene.
I kind of assumed it was all of piece, really. I know the scenes at the beginning of AHBL II must have been awful, but I sort of always assumed he meant this one was, too.
It's a good damn thing they didn't end the season on this episode. I'd have been pi-issed.

Not that I've watched this scene obsessively.
Sam's eyes roll up as he goes to his knees. He breathes out, hard. As Dean reaches him his eyes are open, and after the first cut to Dean's face and back to Sam, Sam's eyes cut to the right. I think that's his last moment of consciousness. He may have taken another breath or two, but his eyes close and he doesn't support his head, or any of his weight at all himself, after that. Dean's talking to a dead man before he gets to "I gotcha."
