Someone on LJ had the idea that the Hunter will do his research and discover how "supernatural" Sam is and go after him. Which I think sounds very, very plausible.
I think it was shocking for Dean, because he couldn't imagine having to put Sam down. Or it might be whatever secret plan the demon has for Sam that Dean now knows might result in him having to kill Sam at some point and he can't imagine doing it without pause or reservation.

Last week's was a snippet of that arguement between Lee and Bill Adama. . . interesting choice. This week's was also an interesting choice. They seem, so far, to be going for more snippets of character stuff rather than actiony stuff.
