I saw a theory on lj that the boy is probably allergic to peanuts and specifically requested peanut butter cookies to prompt a reaction that would require hospitalization - then he escaped from there. That seems most likely.
I suspect that pointedly filmed reveal of the cellmate will have some connection to Josh's escape.

I'm fully on board with the intentional (on Josh's part) allergy trigger theory. I think that shot of the cell mate was probably to show that someone would notice Josh was going into anaphalactic shock from the peanut butter. Poor V. She got hosed.
