The Social Register had a big fit about listing President Jimmy Carter under "Jimmy," even though that was his preferred name. I'm trying to remember, and I htink the only time he was called "James Earl" was when he was inaugerated.

I'm a big fan of the formal name option. I was never supposed to be called Juliana - my parents called me Julie until I was 10, and then I pitched a fit and changed it to Juli. Then, at 17, I decided that that didn't fit me and I went to Juliana.
