Nine polymorphisms within the b2-adrenergic receptor coding block have been described with four resulting in amino acid substitutions at positions 16, 27, 34, and 164 of the b2-adrenergic receptor.33 The three different alleles listed in the table have demonstrated functional differences in vitro. At loci 16 glycine (Gly) and arginine (Arg) can be substituted for each other, at loci 27 glutamine (Gln) and glutamic acid (Glu) can be substituted and at loci 164 isoleucine (Ile) can be substituted for threonine (Thr).46 The Gly/Gly homozygous state is significantly more susceptible to agonist induced down-regulation of the b2-adrenergic receptor than the Arg/Arg homozygous state with the heterozygote state at loci 16 (Arg/Gly) showing an intermediate effect.47 At loci 27 the Glu/Glu homozygote protects against b2 agonist down-regulation combined with Arg/Arg but not with Gly/Gly. Finally, substitution of Thr by Ile resulted in 2- 3-fold decreased binding affinities for agonists to the b2-adrenergic receptor.33 The Ile allele is very rare and appears to be lethal if homozygous.
