Monkey in the middle to me is a particular variety of keep away. Two (or more) tormentors, one object, tossed back and forth over the head of the monkey. Keep away is more of a free-for-all.

See, in these parts, 'monkey in the middle' was more of an actual game. 'Keep Away' was when you had someone's stuff and were actually trying to, well, you know.
