In the DVD commentary for TMD, Minghella says that he initially planned to leave the question up to the viewer, but that he was aware that including the last shot ruined that idea. He knew that it would give the answer that he was really there, but he loved Jamie's sad goodbye so much that he had to include it anyway.

Donnie Darko was better in the original cut with that kind of ambiguity. TMD is richer for including Jamie's story and his sacrifice for her good.

And I think you guys are talking about Maria in West Side Story, not Anita. Although both of them live, but Maria's the Juliet counterpart. And I love the way that movie ends.

I was trying to figure out who Anita's counterpart was (the nurse?) and why she was supposed to die.
