There's an amazing Ask A Manager letter today about someone who stayed strong in refusing to give her current salary and ended up with a 50% increase!
If a place I worked no longer exists as the place I worked (start up bought out and exists in name only - not at all the product we had when I worked there) and is now based in another state - what do I put as the address? address is required.
Whatever you know or can find. (Like, the address where you actually worked would be fine.) It's not like they are going to mail them anything in real life.
