Aside from higher prices, this has led to providers pulling out of the exchanges leaving some with only one or even no providers. That is what the failing part is all about.
Worth noting that the ACA originally included a public option run by the Feds that would have corrected for this so that (especially rural) areas weren't completely at the mercy of the decisions of insurance companies. It was stripped out to appease Republicans.

Jeeeesus, just took a shower (because I work at home and can shower at 2 if I want), and when I popped open the top of my shower gel, some of it flew directly into my right eye. I needed one of those emergency eyewash stations they have in labs.
I settled for turning the shower to cold and sticking my face under it a lot, but it still feels painful.
