That is exactly what my newest freelance gig is going to have me do: they email me PDFs of articles, I print them, mark my changes by hand, scan the marked-up articles, save them as PDFs, and email them back.
Yes, this is in a world in which Adobe Acrobat has a sticky-notes function with which I could note my changes. I don't even. (But they're paying me more than the AMA, including paying me for the time to print out and then re-scan the articles, so I won't complain. Except for the part where it's wasteful as HELL.)
The whole article? Jeesh. I'm pretty sure my case is just that our IS dept (what everyone else calls IT, but we have a different dept. with those initials, so Information Systems is the term we use) hasn't gotten around to making it so we don't have to do that
