Ahhhhh you guys I'm giving notice tomorrow. If I don't chicken out. I'm giving them a month, at least that is the plan. Here's approximately what I'm planning to say:
I just wanted to let you know that I'll be moving on at the end of April. I appreciate all the opportunities you've given me, and I'm ready to take on some new challenges.
What else should I say? I'm sure they will ask questions about what I'm doing next etc, but I don't know what else to kick off with.
As for what I'm doing, I'll be working with a dear friend/mentor three days a week, setting my own hours (HALLELU) and with more responsibility but a more supportive and less pressured environment. It should be a lot easier on my body, too. I will also up my time at my other job from one day a week to two.

smonster, I like what you've said here and I'd add that, beyond this, silence is your friend. There is no need to fill in the blank spaces with more words. My motto is, the more words, the more fear...so NO MORE WORDS!
You are giving them a gift with the 30 day notice. That is all that needs saying.
If they ask about what you are doing next...which you are under no obligation to answer...you might try, "I'm building on the valuable experience I have had here in a position that will be easier on my bones, while meeting several other needs."
They can't argue with you making a choice for your health.
Depending on what time you plan to give your notice, if you'd like to do a walk through of the convo right before, you are welcome to call me!
Are you giving notice in writing? They probably need something printed and dated, to make it all official. (You still have to say something, too, because just grunting and shoving a piece of paper at your boss is probably not the best idea.)
But this is a big step -- I'm excited for you!

Steph, I'm planning on following up with an email but I know my boss values face to face conversations so I'm starting with that.
